Minister's License and Church or Organization Charter
Note* This form is used for several purposes. If used for transfer of License from another denominational church, fill out the form, send supporting documentation from other church and submit appropriate fees.
If the ODB Missions Board requires you to take ministerial certification studies, submit applicable paperwork from other church and choose with certification studies you desire and send appropriate fees.
With application, please send appropriate fee(s). If you are applying for a degree program please go here; Application for Ministry/Courses If applicant is ordained or Licensed in another church, attach copy of ordination or license to the application with appropriate fee(s) and written application.
Fee's (*$18 Annual $5.95 S&H for Ministerial License Transfer & $36 Annual for Our Daily Bread, Missions Church Charter) *Note: If applicant is transferring from a recognized Church or Organization and the applicant does not retain the proper credentials in their current church, applicant will have to go through certification process and enroll in Ministerial Studies .
